Korean National Health Insurance vs Travel Insurance: What Visitors Should Check

Check whether NHIS or travel insurance covers your Korean hospital visit, what approval or referral is needed, and how the hospital will ask you to pay.

In this guide 4 sections
  1. Identify the coverage you already have
  2. Check your NHIS category and current status with NHIS
  3. Check private-policy coverage and how you will pay
  4. Tell the hospital if both types of insurance apply

NHIS is Korea’s national health insurance scheme; travel insurance is a separate policy for your trip. Check whether your NHIS membership is active and which coverage and referral rules apply to the visit. For travel insurance, check your policy’s benefits and any approval needed before care. Ask the hospital how you will pay, including if you hold both types or have no confirmed coverage.

Identify the coverage you already have

Separate your existing insurance status from a policy you are considering buying or an enrolment application that is still unresolved.

Your current positionWhere to check coverageWhat to ask the hospital
NHIS membership confirmedNHIS eligibility and coverage informationThe NHIS pathway for the visit, identification and any referral requirement
Travel or other overseas insuranceThe issuer and the terms of your actual policyWhether direct billing is possible for this service, and what authorization is required

No applicable coverage confirmedResolve any outstanding eligibility or policy questionThe self-pay estimate, payment steps, and documents issued

Save the membership or policy reference, coverage dates, and the insurer’s response with your visit notes. Follow up on any unanswered coverage questions before relying on the insurance.

Check your NHIS category and current status with NHIS

NHIS guidance for foreigners distinguishes employee-insured people, self-employed insured people, and dependents. Ask NHIS which membership category applies to you and whether coverage is active for your visit date. Do not infer membership from nationality, a single length-of-stay rule, or an immigration document without checking your actual status.

For that enquiry, have your current residence and registration information, employment or dependent details, any membership notice, and the proposed visit date ready. Ask which documents are needed to resolve any outstanding issue.

The NHIS Centers for Foreign Residents page identifies eligibility and contribution management as services for insured foreign residents. Use its current jurisdiction information when finding the appropriate office, then resolve your own category and coverage dates through that enquiry.

Check private-policy coverage and how you will pay

Tell the travel insurer which service you plan to use and the visit date. Ask what approval is required and which claim documents you need if you must pay first. Then check that the hospital has received and accepted any authorization needed for billing.

Samsung Medical Center’s billing guide separates its non-NHIS international-patient information from NHIS procedures. Its international-insurance process uses a Guarantee of Payment (GOP); without the required GOP before the visit, its instructions say to pay upfront and claim independently. These are that hospital’s arrangements, not a promise that every travel policy qualifies for direct billing.

Use the travel-insurance hospital-billing guide for the insurer/hospital authorization sequence. A hospital’s ability to process a payment and an insurer’s decision to cover it remain separate questions.

Tell the hospital if both types of insurance apply

Hospitals can handle the combination differently. SNUH’s published instructions say it applies only NHIS at that hospital when a patient also holds global insurance. Samsung’s guide instead directs NHIS-covered patients wishing to use direct billing to contact its International Healthcare Center in advance. Check the policy of the hospital you plan to visit, especially if you hold both types of insurance.

Before the visit, write down:

  1. Your confirmed insurance status for the visit date.
  2. The department and service being requested.
  3. The hospital’s applicable billing procedure and required documents.
  4. Any unresolved authorization, upfront-payment, or claim question.

For a tertiary-hospital visit, check the referral-letter arrangements. If you will pay yourself or some items are outside the confirmed coverage, use the self-pay estimate guide. Request an estimate for the proposed care; insurance membership alone does not mean every item will be paid for.
