Clinic vs Hospital in Korea: Where Should You Start?

Understand Korean clinic and hospital labels, how university affiliation differs from tertiary designation, and which referral requirements to check.

In this guide 4 sections
  1. Understand what the labels mean
  2. Check university affiliation separately
  3. Separate institution type from NHIS benefit rules
  4. What to ask before booking

For a non-emergency first visit in Korea, a clinic offering the relevant outpatient service is one possible starting point. If a clinician has already directed you to a hospital department, check that department’s intake instructions. To narrow your options, check the service offered, the institution type, and any referral requirements for your insurance or self-pay visit.

Understand what the labels mean

The Korean Medical Service Act, Article 3, distinguishes clinic-level institutions principally providing outpatient care from hospital-level institutions principally providing inpatient care. This distinction describes the institution type; it does not mean that hospitals only see admitted patients.

These terms are useful when reading a facility’s official information:

LabelMeaning to checkUseful question
Clinic — 의원An institution principally focused on outpatient careDoes this clinic assess the concern I want to discuss?
Hospital — 병원A hospital category within the hospital-level groupWhich outpatient department or service should I contact?
General hospital — 종합병원A hospital category subject to statutory bed-capacity, department, and specialist-staffing requirementsDoes the department I need accept an initial appointment?
Tertiary general hospital — 상급종합병원A designated general hospital specializing in complex care for serious conditionsWhat referral and registration arrangements apply to this visit?

Articles 3-3 and 3-4 of the Act set out general-hospital requirements and the separate tertiary designation. Use the facility’s current official information to check its status.

Check university affiliation separately

“University hospital” — 대학병원 — is not a separate institution category in the Article 3 classification above. Check the institution’s category and whether it currently holds a tertiary designation; a university name alone does not confirm that status.

The labels also do not tell you the arrangements for a particular appointment. You still need to confirm the department, whether an initial assessment is available, and any required referral or prior records.

Separate institution type from NHIS benefit rules

Korean National Health Insurance has a specific care-benefit process. NHIS describes two phases:

  • The first covers care at institutions other than tertiary hospitals, including clinics, hospitals, and general hospitals.
  • The second concerns care at tertiary hospitals. A doctor’s referral is generally required, with specified exceptions.

This is why calling every clinic “level one,” every hospital “level two,” and every university hospital “level three” can be misleading. In the NHIS explanation, several institution types fall within the same first benefit phase.

NHIS lists exceptions that include emergency care and family medicine, among others. Ask the receiving hospital and NHIS which requirements apply to your appointment. If you are using private insurance or paying yourself, ask what you need for that visit; NHIS rules do not automatically apply to every foreign visitor.

What to ask before booking

Use the situation closest to yours to prepare an enquiry:

Your situationWhat to clarify next
You have not had an initial assessmentDescribe the concern to a clinic or patient-services team and ask whether its service is appropriate for an initial visit
You already have a referralAsk the receiving department which documents to send or bring and how the appointment is confirmed
You want NHIS-covered care at a tertiary hospitalCheck the referral requirement or applicable exception and the expected payment arrangements
You need language supportConfirm how the medical discussion, test instructions, and results will be explained for this appointment

Konkuk University Medical Center’s outpatient guide provides an example: it describes a referral requirement for patients using Korean NHIS and warns that a same-day appointment may not be available. These are Konkuk’s instructions. Check your chosen provider’s own referral and appointment requirements.

Save the reply with the service or department, confirmed date, required documents, language support, and payment instructions. Use these details to prepare for the visit rather than relying on the institution’s category alone.

For the steps from first contact through consultation, payment, and follow-up, use the first-visit guide for foreigners in Korea.

This guide explains institution labels and questions to check. It does not determine the right level of care or insurance entitlement for an individual.
