Medical Certificates and Doctor's Notes in Korea: What Should You Request?

Match a Korean hospital certificate or doctor's note to the recipient's required information, language and form, then confirm consultation and issuance steps.

In this guide 4 sections
  1. Identify what the recipient needs the document to show
  2. Clarify what “doctor’s note” means in this request
  3. Ask for a new certificate or statement during the consultation
  4. Check the document before submitting it

To choose a medical certificate or doctor’s note in Korea, start with the information the receiving school, employer, insurer, or clinician requires. Match that requirement to the hospital’s available document and ask whether a clinician needs to prepare a new statement. Before ordering, confirm that the proposed document meets the recipient’s language, form, and submission requirements.

Identify what the recipient needs the document to show

The purpose of the document determines the question to ask. Keep the recipient’s written instructions or form available for the hospital to review.

Information the recipient requestsDocument question for the hospital
Appointment detailsIs an appointment-confirmation letter sufficient, and which details will it contain?
Confirmation that treatment or hospitalization took placeWhich certificate records the actual visit or treatment period?
Clinical findings or an opinionWhich clinician-issued certificate or statement can address the requested information?
Existing history, test results, or treatment recordsWhich record copies cover the requested dates and information?

SNUH’s document guide illustrates the distinction. Its appointment-confirmation letter lists identification, clinician, specialty, and date/time details. For a letter containing a diagnosis, it directs patients to request a medical certificate from the doctor during consultation. Do not substitute the appointment letter for a clinical statement or assume it confirms attendance.

If the recipient needs existing notes or results, the medical-records request guide explains that process. A new certificate and a copy of information already in the record need separate preparation.

Clarify what “doctor’s note” means in this request

English labels can describe different documents. SNUH’s records page uses “doctor’s note” as a heading covering items such as progress notes and discharge summaries. The SNUH Healthcare System Gangnam Center’s table lists a doctor’s note separately from a medical certificate. It lists Korean and English certificate versions and a Korean version for the doctor’s-note entry.

Give the hospital the recipient’s written requirements rather than requesting a document title alone. Ask what information it can contain and whether its language and format meet those requirements. As the two SNUH examples show, “doctor’s note” can refer to different documents even within one healthcare system.

If the recipient has supplied a form, ask whether the clinician can complete it or whether the hospital issues its own document. Ask the recipient to confirm whether that alternative meets its requirements.

Ask for a new certificate or statement during the consultation

Samsung Medical Center’s documentation guide describes initial certificate issuance through an outpatient consultation in the relevant department. Its document list includes medical certificates, doctor’s statements, and outpatient-treatment or hospitalization certificates; it also specifies payment and photo identification for issuance.

SNUH’s guide asks patients to request an English medical certificate from the doctor during consultation. For a new certificate requested afterward, its guidance calls for another consultation. The Gangnam Center likewise describes the attending physician’s involvement in certificate preparation.

Explain the recipient’s purpose and requested information while arranging the appointment. The clinician determines what can be stated from the assessment and records. A requested outcome, such as a particular work restriction or insurance conclusion, should remain a question for the relevant decision-maker.

If relevant history is held abroad, use the overseas-record preparation guide to organize it for the Korean consultation.

Check the document before submitting it

Use this short note to connect the recipient’s instructions with the hospital’s reply:

Recipient and purpose: [who needs the document and why]

Requested information: [dates, clinical information, or other specified fields]

Form and language: [required form or accepted alternative; language]

Hospital document: [confirmed name, clinician involvement, and any further appointment]

Issuance: [identification, charges, collection arrangements, and expected availability]

Submission: [recipient’s deadline, format, and any authentication requirements to clarify]

Check the issued document against the requirements and ask the issuer about factual errors or missing agreed information. Check with the recipient whether it accepts the document; hospital issuance alone does not confirm acceptance. For an insurance submission, the insurance billing guide helps you match the insurer’s requirements with what the hospital can provide.

This guide helps you choose and request documentation. The clinician, issuing hospital, and receiving institution must confirm the content, issuance arrangements, and acceptance for your case.
